As a new patient we want to provide you with some information to help your first visit go smoothly.
First – Schedule your Initial Evaluation with our front desk staff.
What to bring to your appointment:
* Health Insurance Card(s) – If we are treating you due to a workman’s comp injury or motor vehicle accident bring your adjustors name, contact information and claim number
* Therapy order from your Physician. If you were told it was being faxed to our office, please verify that we have received the form prior to your appointment.
* Picture ID
* Print each of the documents listed below & fill out in BLACK ink
* Wear comfortable clothing and tennis shoes. Preferably something similar as what would be worn to a gym.
* Arrive 15minutes prior to your appointment.
As a courtesy we will verify your insurance benefits for you so that you may know what your cost will be. We prefer to do this prior to the first visit, but in some cases that is not always possible. If you have any questions please feel free to contact our office.
If you are unable to view & print the following forms prior to your appointment. Please arrive 20-25minutes prior to your appointment time to allow you to fill them out in the office.
