Please bring your completed forms along with your insurance card, picture ID & the order for therapy from your physician and arrive 30 minutes prior to your scheduled appointment time. If for any reason you are unable to complete the paperwork prior to your appointment we ask that you arrive 45 minutes early to allow time for the paperwork to be completed in the office.
